#3 Always give the maximum number of hotel choices available. If you only put one or two choices down, by the time we contact you and get the paperwork for more choices you may be down to your 8th or 9th choice rather than your 4th or 5th.

#4 Please complete the information as clearly as possible. Events By Design, Inc. will send you your confirmation by fax or email only. If you do not receive confirmation or a response from us within 72 hours of sending in your paperwork, please contact us directly. There have been several instances where we cannot read a phone number, email address, etc. If you would like to follow up your fax with an email letting us know that you sent in your form, feel free to do so.
